 | Thank you so much for agreeing to be surveyed. Survey results will be used to evaluate the value added by Fair Housing Initiatives Program (FHIP) to society and if program changes are needed to strengthen it.
Your participation and cooperation in this survey is important. Individual responses are confidential. The report that is produced will only present your answers in combination with answers from other FHIP agencies. No responses will be attributed to an individual agency. Fair Housing program staff will not be provided your responses. HUD Policy Development and Research staff will use the information you provide solely for the purpose of the study. Unauthorized disclosure of information by persons conducting the survey is strictly prohibited.
The first step is to create a login ID and password. Once you have created that login ID and password – you will next be asked to provide us with your e-mail address. Once your login ID and password have been approved, an e-mail will be sent to the address you have provided. You can then login to the web survey and begin answering questions.
